TABLET SCREENING VIDEO SCRIPTS
Video No. 1 – General
To serve people across the United States, the National Survey on Drug Use and Health collects data on
substance use, mental health and other health-related issues that are used by local, state, and national
agencies to support education, treatment and prevention programs.
For this survey, households nationwide just like yours are scientifically selected at random and then
visited by a professional interviewer.
During this visit, the interviewer asks a few general questions to determine if anyone in your household
will complete an interview. Those completing the full interview will receive $30 cash.
To protect your privacy, you record most of your answers into a computer. The interviewer never sees
or hears them. Your answers are combined with those from thousands of others, and are reported only
as overall statistics.
You cannot be replaced so your participation is very important.

Video No. 2 – Benefits
Since 1971, the National Survey on Drug Use and Health has collected information on substance use,
mental health and other health-related issues in the United States. Each year, almost 70,000 people
take part in this important study.
Results have shown that millions of people across the country are impacted by the issues of substance
use and mental health each day – people we all know: our family, our friends, our co-workers.
By taking part in this study you may have the opportunity to improve the lives of those in your
community. Local, state and national organizations depend on these data to develop and support critical
education, treatment and prevention efforts. Teachers, school administrators, doctors and other health
care professionals all use data from this study to educate Americans about the dangers of substance
abuse. Researchers and local health departments use this data to track progress and make
improvements in substance abuse prevention and treatment programs. The data can also be helpful in
determining where and when mental health services should be made available to best serve those who
need them.
We each play an important role in creating a better future for our communities, our friends, and our
families, but we can’t do it without your help. Your participation may help ensure that those in need
receive help and education on substance use and mental health issues, which, in the end, benefits us all.

Hello, I’m ________with RTI International. We are conducting a
nationwide study sponsored by the U.S. Department of Health and
Human Services. You should have received a letter about this
study. (SHOW LEAD LETTER, IF NECESSARY.)

ONE YOUTH IS SELECTED:
Your (AGE) year-old child has been selected for an interview. I
would like to talk with him/her to see if he/she is interested in
participating in this study which asks about tobacco, alcohol,
and drug use or non-use and other health related issues.
If your child is interested, I will give both of you more information
and ask for permission to complete the interview. (Is he/she
available?)

TWO YOUTHS ARE SELECTED:
