Explanation of Non-Substantive Changes to OMB Control Number: 3060-0824:

• Instructions to FCC Form 498

Purpose of this Submission: This submission is being made for a proposed non-substantive change request to an existing information collection pursuant to 44 U.S.C. § 3507. This submission seeks to modify: the accompanying instructions to the FCC Form 498.

Summary of Proposed Changes to the FCC Form 498 Instructions:

• Validate Banking Information Clarification: As described in Appendix B, the Universal Service Administrative Company may request additional documentation to validate and confirm the banking (i.e., copy of bank statement and/or voided check). This update clarifies that this documentation may be requested for new or revised FCC Form 498 submissions when changes are made to banking information.

• Reference Term Change: Correcting the term of “applicant” to “billed entity(ies)” throughout the instructions for the FCC Form 498.

• Clarification of Banking Validation Notice: Clarified banking validation notice in the “Financial Institution and Remittance Information” section for each programs. It now reads as follows: In order to protect recipients of Universal Service Funds, USAC may request additional documentation to validate the banking information submitted by filers of the FCC Form 498. Do not provide additional documentation with your submission. You will be contacted by USAC with instructions on how to securely provide this data to validate banking information. Please see Appendix B on page 23 of the instructions for additional information.

• Clarification of Filing Requirement Language for Billed Entities: Slight modification to language under “Filing Requirements and General Instructions” on page 2 of the FCC Form 498 Instructions. Language now reads as following: In addition all Schools and Libraries Applicants (billed entities) receiving payments via the Billed Entity Applicant Reimbursement process must file FCC Form 498 to receive disbursement payments.
