NIFA Veterinary Medicine Loan Repayment Program
Instructions (NEW APPLICANTS): Use 10,000 characters or less including spaces (approximately five
double-spaced, typed pages) to describe your short-term and long-term career objectives in veterinary
medicine, your plan to achieve these objectives, your relevant knowledge, skills, abilities and
experience, and your practice plans and logistics to address the specific shortage situation to which you
are applying. Include an explanation of 1) why you believe the situation you are applying for is one truly
in need of food supply or other specialty veterinary services, and 2) why you believe you are well trained
and professionally positioned to mitigate this shortage situation.
Instructions (RENEWAL APPLICANTS): Use 10,000 characters or less including spaces (approximately
five double-spaced, typed pages) to provide a progress report starting with the time since the start of
your current (or most recent) VMLRP award, including how you are effectively addressing the specific
shortage situation in which you are serving. Include an explanation of why you believe you are well
trained and professionally positioned to continue the mitigation of this shortage situation.
Instructions (ALL APPLICANTS): Please include your name, date, and the page number in the header of
each page of your personal statement. Handwritten information in the header is acceptable, but no
handwritten answers for the personal statement will be accepted.

Public reporting for collection of information is estimated to average 6 hours, including the time for reviewing instructions,
searching existing data sources, gathering and maintaining the date needed, and completing and reviewing the collection of
information. An agency may not conduct or sponsor, and a person is not required to respond to, a collection of information,
unless it displays a current valid OMB control number. Send comments regarding this burden estimate or any other aspect of
