Thank you for your participation in the High Protein (Greek-style) Yogurt Pilot to help us evaluate this potential new product for USDA Foods.  We have prepared an evaluation survey via the online survey tool Wufoo available at the following link:

We ask that you please forward this survey link to your school districts participating in the pilot to provide feedback to FNS on their experience with this product.  We are looking for responses from the districts by [date] and would appreciate any feedback we can get in that timeframe.  Additionally, if you have any questions about the product or the pilot, or any feedback from the State perspective related to the yogurt and/or your interest in ordering this item in the future, please feel free to contact me.
