We are undertaking a survey among Hawaii residents regarding preparedness for natural disasters such as hurricanes. Your name was randomly selected and everything you tell us today will be kept confidential, and your name will never be associated with the responses you give. It should take 30 minutes or less total.

Before we start, I need to provide you some background information. The primary sponsors of this survey are the U.S. Army Corps of Engineers, the Federal Emergency Management Agency, and the Hawaiʻi Emergency Management Agency. A Federal agency may not conduct or sponsor, and a person is not required to respond to a collection of information unless it displays an assigned number from the Office of Management and Budget. The public reporting burden for this collection of information, 0710-0016, is estimated to average 30 minutes per response, including the time for reviewing instructions, searching existing data sources, gathering and maintaining the data needed and completing and reviewing the collection of information. Send comments regarding the burden estimate or burden reduction suggestions to the Department of Defense, Washington Headquarters Services at [email protected]. Respondents should be aware that notwithstanding any other provision of law, no person shall be subject to any penalty for failing to comply with a collection of information if it does not display a currently valid OMB control number.
