We understand that although we cannot control such forces, we can better prepare ourselves, our families, our homes and our communities to weather the winds, rain and high seas, and the damage they can bring.

Enclosed please find a survey; we need your help in completing and returning it at your earliest convenience. The survey is designed to provide emergency management agencies and their partners with information they need to better prepare all of us for hurricanes and major storms.

Please return your completed survey in the enclosed postage-paid envelope addressed to SMS Research and Marketing, Inc. Your participation is completely voluntary. There is no risk to participating. Your individual information will be confidential; the information will be summarized into broad categories and no personal identifying information will be included with the research results.
Agency Disclosure Notice: The primary sponsors of this survey are the U.S. Army Corps of Engineers, the Federal Emergency Management Agency, and the Hawai’i Emergency Management Agency. A Federal agency may not conduct or sponsor, and a person is not required to respond to, a collection of information unless it displays an assigned number from the Office of Management and Budget.
