We need your help!
In a few weeks, you will receive a letter
with information about an important
national survey. The letter will come from
the Census Bureau and will be addressed
to Current Resident. Please read the letter
carefully. A Census Bureau interviewer will
then visit your home to ask you to
participate in the survey.
Your participation is important. Your
household represents thousands of other
homes like yours, so your responses
provide critical information.
Please help us make your voice heard!

Dear Resident,
We recently contacted you about participating in the National Health Interview Survey (NHIS). We
understand you were busy and were not able to complete the survey. We ask that you take the time to
do this important health survey because your household represents thousands of others like yours. We
cannot replace your household with another. The answers you give will be combined with those of
others. Together they will be used to take the pulse of the nation. This is your chance to have your voice
heard on health issues!
To make it easier for you to complete the interview:
•

Your Census Bureau Interviewer will accommodate your schedule and your needs.

•

You can always pause during the interview to attend to other duties. Your interviewer can also
come back at a more convenient time.

•

If a time outside of regular business hours is better, that’s fine! Your interviewer is more than
willing to schedule an interview at a time that works for you. This includes times before 9:00 am
and after 9:00 pm, during the week and on weekends.

The Census Bureau is collecting the information for this survey, on behalf of the National Center for
Health Statistics. In a few days, your interviewer will try to reach you again. At that time, your
interviewer will work with you to find an interview time that is convenient for you. If you would like to
schedule an appointment, please call your interviewer, <> at <>. If you need
more information, please call the Denver Regional Office at 1-800-852-6159, Ext. 23810.
Many families across your community, state, and country have helped their nation by taking part in the
NHIS. Now you have the chance to do the same. Thank you for reconsidering your participation in this
