The New York State Department of Health and the CDC are inviting you to
participate in a survey about tickborne disease and prevention practices in
schools. Your responses will help us to better support you to prevent tickborne
diseases in your students.
This voluntary survey will take about 10 minutes and is completely anonymous.

New York State Department of Health in the
