The Maryland Department of Health and the CDC are inviting you
to participate in a survey about tickborne disease and prevention
practices in schools. Your responses will help us to better support
you to prevent tickborne diseases in your students.
This voluntary survey will take about 10 minutes and is completely
anonymous.
Enclosed you will find a survey to be completed and mailed
back to the Maryland Department of Health in the selfaddressed stamped envelope provided. Please return
