This website will be hosted on the Centers for Disease Control and Prevention TickNET website
(www.cdc.gov/TickNET/survey). A link to this webpage will be sent to participants in a survey invitation as well
as a follow-up reminder. Participants can view this webpage if they are concerned about the legitimacy of the
study or if they wish to reach the study coordinator for the state in which they reside. This webpage is NOT
meant for recruitment purposes.
HPWS Survey
The CDC and two Emerging Infections Program (EIP) sites are conducting a survey to better understand
