Population Assessment of Tobacco and Health (PATH) Study
Thank you for participating in the PATH Study!
You and thousands of others have been a critical part of one of the most important national health
studies ever done in the United States.
What is the PATH Study?
The National Institutes of Health (NIH), in partnership with the Food and Drug Administration
(FDA), is sponsoring this exciting new study on tobacco and health, called the PATH study. The
PATH Study includes people age 12 and older, whether or not they use tobacco. We identified you
or your child during the first year of the study as eligible to participate.
The study will teach us about why and how people are using–or not using–different types of tobacco
products (not just cigarettes) and how use can affect health. Because we want to know why people
do not use tobacco products, including tobacco non-users is critical. The goal is to interview
about 46,000 people at their homes each year of the study, making the PATH Study the largest of its
kind ever done.
The information we collect will be used for research purposes only. The study will keep this
information private; it will only be seen by individuals working on the study.
What are you asking me or my child to do?
If you were 12-17 years of age during the first year of the study, you agreed to participate as a youth.
Now that you are 18, we would like you to agree to participate as an adult.
If your child was 9-11 years of age during the first year of the study, you agreed that we could come
back when he or she is 12 years old. We would like your child to agree to have an interview. Also,
we would like you to have an interview.
Whether or not you agree to continue in the study is up to you. However, the study results will be
much more complete and useful if you participate.
Why should I continue to be part of this study?
You should be part of the PATH Study because the results may help to improve the health of
millions of Americans.
Whether or not you use tobacco, you will be an important part of what is learned about tobacco
products and how they affect health. Your participation can make a difference.

What happens next?
•

If you are now 18, we will ask you to sign a consent form and take part in an interview.

•

If your child is now 12 years old, we will interview him or her only if a parent or guardian gives
permission, and the youth agrees to participate.

How will you protect my privacy?
All the information you give to the PATH Study will be kept private.
•

Your answers will not be connected with your name.

•

We will identify your answers by a number only.

•

Your name and contact information will be kept separately in secure computer files.

Will I get anything for being in the PATH Study?
Yes. For adults, the PATH Study offers as a thank you up to $85 for fully participating in both of
two home visits.
For youth, the PATH Study offers as a thank you $25 for completing an interview. Parents are
offered $10 for completing an interview about their youth.
How can I get more information about the PATH Study?
The PATH Study interviewer can answer any questions you have about the study.

¿Qué es el Estudio PATH?

this Certificate is issued in response to the request of the Principal Investigator, Andrew Hyland, Ph.D.,
Behavioral Health Group, Westat, 1600 Research Boulevard, RB 4121, Rockville, MD 20850-3129, to
protect the privacy ofresearch subjects by withholding their identities from all persons not connected with
this research. Dr. Hyland is primarily responsible for the conduct of this research, which is funded by NIDA
contract# HHSN271201100027C.
Under the authority vested in the Secretary of Health and Human Services by section 301(d),
all persons who:
1. are employed by or associated with Behavioral Health Group, Westat and its research sites,
contractors, or cooperating agencies and
2. have in the course of their employment or association access to information that would
identify individuals who are the subjects of the research project known as “Population
Assessment of Tobacco and Health or PATH Study,”
are hereby authorized to protect the privacy of the individuals who are the subjects of that research by
withholding their names and other identifying characteristics from all persons not connected with the
conduct of that research.
The research began on September 19, 2011 and is expected to end on September 30, 2017.
The purpose of this national, longitudinal cohort study is to examine the epidemiological and health
consequences of tobacco-product use among the U.S. civilian, non-institutionalized population ages 12
years and older.
Study subjects’ identities are protected by linking unique study ID numbers assigned to sampled households
and individuals with personal identifiers. Survey data with study IDs are separated from materials with
personally identifiable information (PII) through data collection, transmission, storage, processing, analysis,
and reporting. Data are collected on encrypted laptop computers. Study data are identified and retrieved by
study ID numbers only. Hard copy consent forms with participant names and all PII are maintained in secure

areas separate from the research data. They are identified only by study ID number and are stored in locked
files. Biospecimens are coded and shipped for analysis by qualified laboratories. No reports or analysis files
will contain PU. Individuals will not be identified in any publication or presentation, and analytic findings
will be reported as aggregate statistics only.
A Certificate of Confidentiality is needed because biologic samples and sensitive information concerning
study subjects’ drug and alcohol use, and sexual orientation/attraction is collected during the course of the
study. The Certificate will help researchers avoid involuntary disclosure that could expose subjects or their
families to adverse economic, legal, psychological and social consequences.
As provided in section 301(d) of the Public Health Service Act 42 U.S.C. 241(d):
“Persons so authorized to protect the privacy of such individuals may not be compelled in any
Federal, State, or local civil, criminal, administrative, legislative, or other proceedings to identify
such individuals.”
This Certificate does not protect you from being compelled to make disclosures that: (1) have been
consented to in writing by the research subject or the subject’s legally authorized representative; (2) are
required by the Federal Food, Drug, and Cosmetic Act (21 U.S.C. 301 et seq.) or regulations issued under
that Act; or (3) have been requested from a research project funded by NIH or DHHS by authorized
representatives of those agencies for the purpose of audit or program review.
This Certificate does not represent an endorsement of the research project by the Department of Health
and Human Services. This Certificate is now in effect and will expire at the end of September 2017. The
protection afforded by this Confidentiality Certificate is permanent with respect to any individual who
participates as a research subject (i.e., about whom the investigator maintains identifying information)
during any time the Certificate is in effect.

URINE

Collection Instructions

As part of the PATH study, we ask you to collect a sample of your urine. Please follow these instructions.
1

Wash your hands with soap and water.

Lay the paper towel in the kit on a flat surface. Take
the lid off of the collection container and place it on the
paper towel. The inside of the lid should be placed face
down on the paper towel.

Don’t touch the inside of the lid or the inside of the
collection container. Don’t allow the inside of the lid or
the inside of the collection container to come in contact
with any part of your body, clothing, or bathroom surfaces.

Urinate directly into the container. Collect all of your
urine; don’t let any of it go into the toilet unless the
container is full.

Replace the lid on the collection container immediately
so that the urine is only briefly exposed to the air.

Use the paper towel to wipe any urine from the outside
of the container.

Wash your hands with soap and water after collecting
the sample.

Give the collection container back to the interviewer.

Frequently Asked Questions
Why are you collecting blood?

Who is Hooper Holmes?
• Hooper Holmes is a national provider of in-home blood collection services.
• For the PATH Study, Hooper Holmes health professionals will visit homes
to collect blood samples from consenting adults.

• All health professionals are trained and certified to draw blood.

The blood collection is critical to the success of the PATH Study.
Researchers will conduct tests on the blood that will help us to understand
how tobacco affects the body. We are collecting samples from adults who
use tobacco and those who do not.
How much blood will you take?

We’ll collect six tubes of blood that equal about three tablespoons.
The body makes blood daily and replaces this amount of blood within
24 hours.
Is it safe?

What Should I Expect?
Step 1
A Hooper Holmes health professional will call you 1 to 2 days before your
scheduled appointment. The health professional will confirm:

• The date of your appointment,
• The time of your appointment, and
• Your address.

Yes. All PATH Study health professionals have done more than
150 successful blood collections and have been trained and certified on
PATH Study collection procedures. The supplies used for blood collection
are completely sterile and are used only once. You can’t be infected by
any bloodborne disease, such as hepatitis or AIDS, through this blood
collection.
Will I receive the results of the tests?

No. The results of the tests will be used for research purposes only, so we
won’t send individual test results to you.

Step 2
A Hooper Holmes health professional will come to your home on
the appointed date and time. The health professional will do the following:

• Show a driver’s license and PATH Study identification,
• Collect information about your recent use of tobacco, and
• Complete the blood collection in your home.

• Ask the interviewer or health professional any questions you may have about
the blood collection or

• Call the PATH Study toll-free number, 1-888-311-1819, weekdays between

As a thank you for participating in the health professional visit, $25 will be
added to your debit card within 3 business days after sample collection.

Health (NIH), in partnership with the Food and Drug Administration (FDA), is sponsoring this
study. Researchers and policymakers will use the information collected in the study to help improve the
Nation’s health.
Last year, participated in the study and completed an interview at home. For the planned
follow-up interview this year, we tried to reach at the residential address and learned that
