Purpose of this Submission: This submission is being made for a non-substantive change request to existing information collections pursuant to 44 U.S.C. § 3507. The Commission seeks to change the Instructions to FCC Form 474 (3060-0856), Instructions to FCC Form 479 (3060-0853), FCC Form 499-A and Instructions (3060-0855), FCC Form 499-Q and Instructions (3060-0855), Instructions to FCC Form 498 (3060-0824), Instructions to FCC Forms 507, 508 and 509 (3060-0986).


Summary of Updates to the FCC Forms and Instructions


  • Address Change: Starting July 12, 2016 the new address for the Universal Service Administrative Company (USAC) will be 700 12th Street, NW, Suite 900, Washington, DC 20005. All of the above listed forms and instructions will be updated with the new address.

  • There are no changes to the burdens hours or cost to this information collection.
