Purpose of this Submission: This submission is being made for a non-substantive change request to existing information collections pursuant to 44 U.S.C. § 3507. The Commission seeks to change the Instructions to FCC Form 474 (3060-0856), Instructions to FCC Form 479 (3060-0853), FCC Form 499-A and Instructions (3060-0855), FCC Form 499-Q and Instructions (3060-0855), Instructions to FCC Form 498 (3060-0824), Instructions to FCC Forms 507, 508 and 509 (3060-0986).

Summary of Updates to the FCC Forms and Instructions

• Address Change: Starting July 12, 2016 the new address for the Universal Service Administrative Company (USAC) will be 700 12^th Street, NW, Suite 900, Washington, DC 20005. All of the above listed forms and instructions will be updated with the new address.

• There are no changes to the burdens hours or cost to this information collection.
