This submission is being made for a
non-substantive change request to an existing information
collection pursuant to 44 U.S.C. § 3507. The Commission seeks to
change the Instructions to FCC Form 498 (3060-0824). The New
Address for the Universal Service Administrative Company (USAC),
starting July 12, 2016, will be 700 12th Street, NW, Suite 900,
Washington, DC 20005. The above listed instructions will be updated
with the new address. This submission is being made for proposed
non-substantive change to an existing information collection
pursuant to 44 U.S.C. § 3507. This submission seeks to modify: the
accompanying instructions to the FCC Form 498. This submission is

accompanying instructions to the FCC Form 498. The Administrator of
the universal service support mechanisms must obtain contact and
remittance information from service providers participating in the
federal universal service High-cost, Low-income, Rural Health Care,
and Schools and Libraries programs. The Administrator uses FCC Form
498 to collect service provider name, phone numbers, other contact
information, and remittance information from universal service fund
participants to enable the Administrator to perform its universal
service disbursement functions under 47 CFR Part 54. FCC Form 498
allows fund participants to direct remittance to third parties or
receive payments directly from the Administrator.
