LENDER’S AGREEMENT

This form is to be used by lenders to meet certain conditions preceding issuance of a Loan Guarantee.

Lenders submit the original of the completed form in hard copy or facsimile to the appropriate USDA servicing office.

Lenders must carefully read the statements in Parts B through I and complete Items J1 and J3.

Part A Background Information is for FSA only.

Part B - I Certifications and Items J1-J3

J-1 Name and Title of Lender Representative	Enter name and title of the lender’s representative.
J-2 Authorized Lender Representative Signature	The authorized representative of the lender must sign document.
J-3 Date	Enter date Authorized Lender Representative signs document.
