K-BAP PHONE RECRUITMENT SCRIPT

Common intro for all informants:

Hello, this is {INTERVIEWER} calling on behalf of the Centers for Disease Control and Prevention. This call is regarding a study of the high HIV diagnoses in {CITY NAME}. In response to this HIV crisis, the CDC is surveying providers in {CITY NAME}. May I speak to {PROVIDER NAME}?

If Provider is on phone:

Several weeks ago, we emailed you about participating in a CDC survey about HIV prevention and treatment in {CITY NAME}. This is in response to the very high incidence of HIV in {CITY NAME}. We’re offering a cash token of appreciation and free continuing education access. Do you recall receiving that email?

If YES: That’s very good. Will you be able to assist us by agreeing to participate? If so, we will mail you your $20 cash token of appreciation and email you a link to the survey, which will take less than 30 minutes to complete. Participation is voluntary and results will be kept strictly private.

If NO: I understand. Will you be able to assist us by agreeing to participate? If so, we will mail you your $20 cash token of appreciation and email you a link to the survey, which will take less than 30 minutes to complete. Participation is voluntary and results will be kept strictly private.

If AGREE: Thank you so much – your assistance is critical and very much appreciated. Can I confirm your email address is {PROVIDER EMAIL}?And can I confirm your mailing address is {MAIL ADDRESS}? We will mail you your cash and email you the survey in the next few weeks.
