K-BAP FOLLOW-UP SURVEY EMAIL REMINDER



Several months ago, we asked you to assist us with the K-BAP [City Name] Primary Care Provider HIV Survey. You graciously accepted our invitation and completed our online survey. For that, we thank you very much for assisting us. As mentioned in the original invitation, we are asking for your assistance one last time by completing a follow-up survey so that we can conduct in-depth analysis on the HIV prevention and care among primary care providers in the southeastern United States.

If you have not had an opportunity to complete it, we are asking for less than 20 minutes of your time, the time it will take you to answer the questions in your survey.
