Pin Instructions

A few days ago, you should have received a debit card as a token of our
appreciation for your participation in the Survey of Income and Program
Participation. The PIN is the last 4 digits of the 16-digit card number. We
suggest that you use this card at an ATM within 60 days of receipt and
that you withdraw the entire amount at one time. You may use the card
immediately.
If you have any difficulty cashing the debit card, please call
1–888–369–3608.

For security reasons, our field representatives do not carry incentive debit
cards on their person.
