Crosswalk for Changes to Continuity Assessment Record and Evaluation (CARE) tool for use in
Functional Assessment Standardized Item (FASI) Set
The TEFT FASI items are primarily a subset of the CARE item set. Only four sections of the CARE tool are
included in the FASI Item Set:
• a subset of Section I. Administrative Items (Gender, Age)
• a subset of Section V. Impairments, (only item H. Mobility Devices and Aides Needed)
• a subset of Section VI. Functional Status (excludes tube feeding, wheeling long and short ramps,
using public transportation)
• a subset of Section VII. Discharge status, including a modified item on frequency of assistance
needed at discharge, living arrangements, and Support Needs/Caregiver Assistance.
A few new items were included in the function section that measure mobility in the community,
including ability to walk/wheel for 15 minutes, ability to cross street before light changes, ability to
walk/wheel around obstacles in rooms, and ability to carry something in both hands while walking. A
set of personal priority items were also included in each subsection (self-care, mobility, instrumental
activities of daily living, and living arrangements/caregiver assistance needs). The priority items are two
open-ended items to allow the individual to identify their priorities for the next six months in each
subsection. Last, the first section in the FASI item set is much shorter, collecting only information on
gender and age (modified from date of birth in CARE) and adding an item on which waiver the person is
on as well as assessor and participant ID numbers. The following describes these modifications by
Section.
