Minor Revisions to the Collection Instrument

SSA is making the following revisions:

• Change #1: We are revising the Privacy Act Statement on Form SSA-1002.

Justification #1: SSA’s Office of the General Counsel is conducting a systematic review of SSA’s Privacy Act Statements on agency forms. As a result, SSA is updating the Privacy Act Statement on the form.

• Change #2: We are revising the Privacy Act Statement on Form SSA-1003.

Justification #2: SSA’s Office of the General Counsel is conducting a systematic review of SSA’s Privacy Act Statements on agency forms. As a result, SSA is updating the Privacy Act Statement on the form.

• Change #3: SSA intends to provide Forms SSA-1002-F3 and SSA-1003-F3 in fillable PDF files on our website, in place of the non-fillable PDF versions we currently provide. We propose to make all form entries fillable.
  
  Justification #3: This change will allow the respondents to continue to print and complete the form by hand, or, alternatively, complete the form using a computing device, such as a personal computer or handheld (mobile) device, print, and submit the form to SSA for processing.
  
  
  

SSA will implement the changes above upon OMB approval.

These actions do not affect the public reporting burden.
