Hello. Could I please speak to <<Parent/Guardian Name>>?

Hi. My name is <<Researcher Name>>. I’m calling from Emory University where we are conducting a study about transition from pediatric to adult care for adolescents with congenital heart defects. I understand from <<Clinic Name>> that your adolescent child will be attending the clinic this week and wondered if you would be interested in taking a short survey.

The survey will take 20 minutes or less and will ask you about your knowledge and concerns about the transition for your child from pediatric to adult specialty cardiac care. There will also be a chance for you to share what information you would like to receive on this topic and how you would like to receive it. And, at the end of the survey, we will offer you a small gift card incentive to thank you for your participation.

  • If Parent/guardian says they are interested:

Wonderful. There are 2 options for taking the survey. I can either email you a link and you can take the survey online at home. Or, if you prefer, you can take the survey in the clinic waiting room when you come for your appointment on <<Scheduled Date>>. Which option would you prefer?

  • If they prefer online:

Sure. Can you please provide an email address that I can send the link to?

  • If they prefer in the clinic:

Sure. When you arrive for your appointment later this week, a researcher will approach you in the clinic with an Ipad so that you can complete the survey then.
