The Health Department of the State of [XXX] in collaboration with Centers for Disease Control and Prevention (CDC) is conducting a pilot of a web-based version of the Behavioral Risk Factor Surveillance System (BRFSS) which has been conducted by state health departments since 1984. The purpose of this information collection is to understand the impact of moving from a telephone based interview to an online questionnaire. We would like for your participation in this questionnaire. The questionnaire will include questions on health risk behaviors (such as smoking) and preventive measures (such as health screenings). There will also be general health questions. You have been randomly selected to participate in the online version. This is a voluntary survey, and you may skip any questions you wish.
