Dear Dr. [enter name]:

On behalf of March of Dimes, RTI International is conducting interviews with women enrolled in the US Zika Pregnancy and Infant Registry to assess the kinds of care their baby is receiving, challenges they may face in caring for their baby, and barriers to receiving health care and support services. This project is being funded by the Centers for Disease Control and Prevention (CDC). Results from this survey will be used to identify the types of services that Zika-affected pregnant women and children need, and work with national, regional, and local partner organizations to fulfill these needs.

I am writing to let you know that your patient[s] [xxx] is included in the sample of women who will be invited to complete an interview about their experiences. We received her name from the state or local health department as part of the US Zika Pregnancy and Infant Registry. The study is completely voluntary and the interview will only take place if she is interested in participating.

This letter serves to inform you about the study; no action is needed on your part.

RTI International is conducting these interviews on behalf of the CDC and March of Dimes. If you have any questions about the study, you can call the project director, Dr. Linda Squiers, at 1-800-334-8571, ext. 25128 (toll free).

Sincerely,

[Health Department Contact]
