Data elements required for release from Quarantine
Written notice that 31-day quarantine period, including any required extension of quarantine, has been completed.
Written notification of the health status of the NHPs in the shipment from the quarantine facility’s licensed veterinarian
Written confirmation that the importer has addressed and resolved to HHS/CDC’s satisfaction any NHP or worker communicable disease issues that were reported to HHS/CDC during shipment.
