Introduction

ICF International is working with the Centers for Disease Control and Prevention (CDC) and the President’s Youth Fitness Program (PYFP) to learn more about physical education and physical activity opportunities for middle schoolers.


You are being asked to take this survey because your class was picked by your school. We would like to know more about student physical activity and fitness attitudes and habits. Your answers are very valuable to us. The survey is short – it should take no more than 15-20 minutes of your time. You do not have to do the survey if you do not want to, and you may skip any question you do not wish to answer. All your responses will be kept private.


Please let the school liaison know if you have any questions while taking the survey. Once you have finished, please turn the survey in to the school liaison who gave out the survey.
