I am writing to invite you to participate in the PYFP Evaluation School Administrator Survey. The PYFP has partnered with the US Centers for Disease Control and Prevention (CDC), the Robert Wood Johnson Foundation and ICF International to conduct this important program evaluation in a small number of schools nationally. The PYFP provides a model for fitness education within a comprehensive, quality physical education program and we are eager to assess the impact the program has on PE teachers, their schools and their students. It is the mission of the PYFP to provide a model for fitness education that includes all of the resources needed to empower students to live healthy and active lifestyles.

We are asking school administrators to complete a brief survey to learn more about policies and environmental supports for physical education and physical activity at your school. Information collected through the Evaluation will be used to assess the impact of the PYFP and to inform program improvement. Program evaluation procedures are designed to protect participant privacy and allow for confidential participation. Results from this evaluation will only be presented in aggregate; participating districts, schools, teachers, and students will not be identified in any published reports.
