1110-0046 Supplementary The FD-258 was previously submitted with the following errors inadvertently miss during the final submission. On page one, a top line should have been added beneath the form number over to the darken line before FBI. Also, there should have been a line between Aliases AKA and the ORI portion of the card. Finally there should have been a line between Citizenship CT Z and Sex on the front page. On the second page the Privacy Act Statement should only have the Authority, Principal Purpose, and Routine Uses listed.
