Revisions to the Information Collection
Change # 1: Revising the language “His (Her)” to “Worker’s” Social Security Number.
Justification: Changing “His (Her)” to “Worker’s” for consistency on the form.
Change #2: We are revising Question 4 to: “To your knowledge, were (are) the Worker and Applicant generally known as a married couple?”
Justification #2: In recognizing same-sex couples, we now use gender-neutral language acknowledging that a question or a statement on a form could refer to two same-sex individuals in a marriage. Changing “husband and wife” to “a married couple.”
Change #3: We are revising Question 5 to: “Did you consider them a married couple?”
Justification#3: In recognizing same-sex couples, we now use gender-neutral language acknowledging that a question or a statement on a form could refer to two same-sex individuals in a marriage. We are changing “husband and wife” to “married couple.”
Change #4: We are revising Question 6 to: “Did you hear them refer to each other as a spouse?”
Justification #4: In recognizing same-sex couples, we now use gender-neutral language acknowledging that a question or a statement on a form could refer to two same-sex individuals in a marriage. Changing “husband and wife” to “a married couple.”
Change #5: We are revising Question 7 to: “In your opinion, did (do) they maintain a home and live together as a married couple?”

Change #6: We are revising the PRA statement on this form.
Justification #6: We are revising the PRA statement to reflect our current boilerplate language. The current language, which dates back to the last reprint of the form, is now outdated.
Change #7: We are revising the Privacy Act Statement on this form.
Justification #7: SSA’s Office of the General Counsel is conducting a systematic review of SSA’s Privacy Act Statements on agency forms. As a result, SSA is updating the Privacy Act Statement on the form.
