Title V Maternal and Child Health (MCH) Block Grant

Jurisdictional MCH Survey Instrument

Are you the mother of a child under 18 years old?

We are testing a survey to help better understand the health of mothers and children in [American Samoa, Federated States of Micronesia, Marshall Islands, Northern Mariana Islands, Palau]. You may qualify for this study if you are over the age of 18 and a mother of at least one child aged 0-17 years living in the same household.

Participation includes one in-person interview about your and your children’s health. It will take about 40 minutes. Participation is voluntary. There are few risks or benefits to you.
