Dear oilfield worker,



The National Institute for Occupational Safety and Health (NIOSH) will be here at ______________________ (Location) on ____________________(Date) to do a research study on safety and health hazards of workers in the oil and gas extraction industry. We would like to invite you to participate in this study by doing a safety and health survey and short interview about your job. All workers at this location will be asked to participate. You need to have worked in the oilfield for at least one month and go to well sites 2 times per week or more to participate in the study.

The mission of NIOSH is to improve worker safety and health through research and education. NIOSH does NOT enforce any laws. We plan to share the results of the study to help improve worker safety and health.

Your participation is voluntary. If you choose to participate, the survey should take you about 30-40 minutes to complete and there will be a short interview at the end. We will provide you with a token of appreciation for completing the survey.

There is minimal risk to participation in this survey. Your answers will be anonymous. The benefit to you is that we will provide you with relevant safety and health information at the end of the survey.
