Addendum to Supporting Statement for Form SSA-783

Revisions to the Information Collection

SSA is making the following revisions:

• Change # 1: We are revising Question 3(a) to remove natural and adopting parents from parenthesis.

Justification 1: We are revising, because the word parent covers all parents except stepparents.

• Change #2: We are revising question 3(b) to add Mother/Father.

Justification #2: In recognizing same-sex couples, we now use gender-neutral language that would acknowledging that a question or a statement on a form could refer to two same-sex individuals in a marriage. Changing “Mother” to “Mother/Father” and “Father” to “Mother/Father”.

• Change #3: We are revising question 3(d) to add Mother/Father

Justification #3: In recognizing same-sex couples, we now use gender-neutral language that would acknowledging that a question or a statement on a form could refer to two same-sex individuals in a marriage. Changing “Mother” to “Mother/Father” and “Father” to “Mother/Father”.

• Change #4: We are revising the PRA statement on this form.

Justification #4: We are revising the PRA statement to reflect our current boilerplate language.  The current language, which dates back to the last reprint of the form, is now outdated.

• Change #5: We are revising the Privacy Act Statement on this form.

Justification #6: SSA’s Office of the General Counsel is conducting a systematic review of SSA’s Privacy Act Statements on agency forms. As a result, SSA is updating the Privacy Act Statement on the form.
