As part of the disability evaluation process, Section 2(a)(3) of the Railroad Retirement Act requires
the Railroad Retirement Board (RRB) to review any claims of employment that may impact a
disability annuity. Our records indicate that you are **CHOOSE**. Please complete the enclosed
Form G-252, Self-Employment/Corporate Officer Work and Earnings Monitoring, and return it
within 30 days from the date of this letter to the RRB in the envelope provided.
After our review you may be asked to submit individual tax returns, corporate tax returns, corporate
minutes and resolutions, a bill of sale, a Limited Liability Corporation (LLC) contract, articles of
incorporation or transfer documents to substantiate your statements.
