Recently, we sent you a short survey about your experience in the Montana Health and Economic
Livelihood Partnership (HELP) plan. Your answers will help us improve the health care and services
provided. The survey will also help other people learn more about HELP.
After you finish the survey, please return it in the pre-paid envelope that you received with the
survey. We will send you a $10 Visa® gift card to thank you for your participation.
If you did not get the survey, misplaced it, prefer to complete it online, or would like to
complete the survey over the phone, please call the survey help desk at the toll-free number
