I-Corps Survey Screener Questions:
The screener appears in the omnibus survey instrument (appendix A, questions 1-3a, with
indication that it only applies to non-I-Corps PIs as the respondent group) and in the non-I-Corps
PI instrument (appendix c, questions 1-3a) – but not in appendix B as it does not appear in the
survey to I-Corps Teams members.

The omnibus instrument below shows all the questions across the survey groups of this ICR. It
shows common questions for the I-Corps and non-I-Corps participants, as well as questions that
are unique to each of these two respondent groups. The second column shows which respondent
group(s) the question applies to. To facilitate review, we show only questions for I-Corps
respondents in Appendix B. Finally, Appendix C shows only questions for non-I-Corps PI
respondents.
