The U.S. Department of Agriculture's Animal and Plant Health Inspection Service (APHIS), the State of _______________, and
the Producer hereby enter into this National Animal Health Monitoring System (NAHMS) Beef 2017 study PRODUCER
AGREEMENT, the terms of which are set forth below.
1.
APHIS and/or the State of __________________ will provide personnel who will be referred to as the Data Collector. The
Data Collector and the Producer will participate together in implementing a statistically valid NAHMS study for determining
national estimates of beef-health practices and for compiling health information to enhance beef production. The Data
Collector will complete one person-to-person interview with the Producer.
2.
The Producer will assist APHIS by providing accurate information regarding beef-health and management practices
related to the study objectives. The Producer retains the right to refuse any questions deemed inappropriate.
3.
The Data Collector will keep the origin of the data confidential by recording the data with the Producer’s unique code number
only. The Data Collector will not keep any key to the code after the completion of the study. The Data Collector and all other
project personnel acknowledge that the Producer is providing information that he/she does not customarily share and is
providing it with the expectation that it will not be made public. The one exception to data confidentiality is the suspicion or
diagnosis of a dangerously contagious, infectious, or exotic disease foreign to the United States on the Producer’s premises
(e.g., foot-and-mouth disease), in which case further investigation and possible action may occur.
4.
Data collected by the Data Collector will not be used for regulatory purposes. However, information on a Producer’s
animals revealed from sources unrelated to the Beef 2017 study, such as testing and inspection for movement or sale of
animals or tracebacks on testing done at slaughter, may cause regulatory action to be initiated by the State or APHIS.
5.
APHIS may publish, or authorize others to publish, the aggregate (summary) findings acquired from NAHMS for the benefit
of the beef industry, private industry, and other interested groups, but will ensure that the identity of the Producer is withheld.
APHIS may not publish, or authorize others to publish, individual responses. APHIS may perform additional testing, or
authorize others to perform additional testing of samples collected through the study, for the benefit of the beef industry, but
will ensure that the identity of the Producer is withheld.
6.
The Producer can obtain any further information available from this study by accessing the NAHMS Web site at
www.aphis.usda.gov/nahms or by subscribing to the NAHMS beef mailing list. Hard copies of the reports can be requested on
the NAHMS Web site.
7.
Any changes to or waivers of the terms of this PRODUCER AGREEMENT shall be binding on APHIS and the STATE of
___________________ and the Producer only if they are put in writing by each party.
8.
The effective data collection period of this PRODUCER AGREEMENT shall begin with today’s date of ____/____/____ and
end no later than December 15, 2018.

According to the Paperwork Reduction Act of 1995, an agency may not conduct or sponsor, and a person is not required to
respond to, a collection of information unless it displays a valid OMB control number. The valid OMB control number for this
information collection is 0579-0326. The time required to complete this information collection is estimated to average .17
hours per response, including the time for reviewing instructions, searching existing data resources, gathering and
maintaining the data needed, and completing and reviewing the collection of information.

9.
Biologic Sampling by Producer—BVD Testing
The Producer can elect to have his/her entire calf crop tested for persistent infection with bovine viral diarrhea (BVD) virus.
To be eligible to participate, 70% or more of the operation’s calves need to be born between November 1, 2017, and June 30,
2018. Samples should be collected only from calves born during this time. Supplies and shipping costs will be covered by
NAHMS. The Producer will take two ear notches from each calf, place them in a plastic bag, write the calf ID and age on the
bag, and then place in a freezer until shipment. The Producer will need to be able to deliver the samples in one or two batches
to a local Federal Express location that accepts Express packages. The Producer will also need to allow his/her contact
information (name, address, phone number, and email) to be available to a member of the NAHMS staff in Fort Collins, CO,
so that collection materials and results can be sent to the Producer.

10. Biologic Sampling by Data Collector—Forage
The Producer can elect to have nutrient analysis performed on one forage sample. Samples will only be taken from baled hay,
not from haylage or silage. The forage selected for testing should be from the same field and cutting number (e.g., second
cutting from a specific field—this will be referred to as a “lot”). The Data Collector will use a forage probe (bale corer) to
collect approximately 20 cores from the forage. Twenty cores produce in total about a half-pound sample. Multiple bales will
be sampled from the same lot as long as the bales are accessible for sampling. Results will be returned to the Producer.

collect one forage sample from baled hay chosen by the Producer.

11. Biologic Sampling by Data Collector—Fecal Bacteria
A limited number of farms in each State will be selected to have fecal samples collected and tested for pathogens. If selected,
the Producer consents and authorizes the Data Collector to collect 10-20 samples of fecal material from the ground. All
samples will be tested for Salmonella, and the Salmonella results will be returned to the Producer as positive or negative. A
subset of samples will be tested for various pathogens including, but not limited to, E. coli, and Enterococcus. These results
will not be returned to the Producer.

the Producer hereby enter into this National Animal Health Monitoring System (NAHMS) Beef 2017 BVD Producer Agreement,
the terms of which are set forth below.
1.
To participate, the Producer must:
a. Have a herd in which 70% or more of calves are born from November 1, 2017, through June 30, 2018.
b. Follow the instructions provided with the collection materials (kit). This includes collecting two ear notches from each
preweaned calf born from November 1, 2017, through June 30, 2018, placing the two ear notches in an individual bag, and
writing the calf ID and age on the bag. The bags will be prelabeled with your operation number.
c. Freeze the samples the day of collection and keep frozen until shipped to the laboratory.
d. When ready to ship the samples, follow the shipping instructions included with the Kit.
e. Use Federal Express to ship samples to the laboratory. Kits should only be shipped on a Monday, Tuesday, or Wednesday.

By signing this Agreement, the Producer is allowing his/her name, address, phone number, and email address to be given to a
member of the NAHMS staff for the purpose of sending the kit and test results directly to the Producer and to contact the
Producer if a problem arises with the samples. NAHMS will maintain this information in a confidential manner, use only for
the purposes described, and will destroy the data when the study is over.

Kits containing the collection materials, including the ear notcher, prepaid shipping labels, icepacks, sample bags, boxes, and
instructions, will be provided by the NAHMS study program. Results will be sent within 2–3 weeks after testing is completed.
4.
How many calves are expected in the calf crop from November 1, 2017, through June 30, 2018? ______________________

allowable for shipping samples to the laboratory is July 16, 2018.
Producer will take ear notch samples all at one time (i.e., when calves are processed as one group). Enter the approximate

6.
By signing below, the Producer agrees to participate in the BVD testing and acknowledges that if he/she does not collect and
ship the samples as instructed, the laboratory’s ability to test the samples may be compromised.
