Parental Contact Information
After the Respondent Selection Screen and parental consent popup, if at least one adolescent respondent
is selected, the following module will appear.
Your [AGE] (child/children) (has been/have been) selected to take part in an important study for the U.S.
Department of Health and Human Services called the National Mental Health Study (NMHS).
Knowledge gained from this study will improve our ability to describe and understand mental health
issues in the United States. RTI International is conducting the NMHS on behalf of the Substance Abuse
and Mental Health Services Agency (SAMHSA) and the National Institute of Mental Health (NIMH).
Both of these agencies are part of the U.S. Department of Health and Human Services.
We would like you to take part in the National Mental Health Study Parent Interview to answer questions
about [IF ONE ADOLESCENT SELECTED FILL: child; IF TWO ADOLESCENTS SELECTED FILL:
children]’s health, development and family background. A parent’s perspective is very important in
getting an accurate description of a child’s health and development so we hope you’ll agree to participate.
You will receive $30 as a token of our appreciation for completing the survey. It will take approximately
30 minutes to complete. If possible, please complete the interview in the next three days.
HAND PARENT STUDY DESCRIPTION TO RESPONDENT. Please read this statement. It describes
the survey and the legislation that assures the confidentiality of information you provide.
