INSTRUCTIONS:

• Report total quantity of seed potatoes purchased or under contract for future from potato growers.

• Include seed potatoes grown outside your state.

• You may report either average price paid per unit or total dollars paid.

• You may report quantity of seed purchased and prices paid using hundredweight (cwt) or barrels (bbls) as the unit of measurement.

• Include transportation and handling charges.

• Exclude sales tax.
