TITLE OF INFORMATION COLLECTION:

HRSA Web Survey for Bureau of Health Workforce (BHW) program participants

PURPOSE:

The goal of this survey is to gain better insight into the telehealth service provisions and other telehealth use of BHW Program participants at rural health centers. The objective is to understand:

• How telehealth is being used in rural health centers,

• How much telehealth is being used, and

• What telehealth technologies are being used.

The information will be used to help inform future training and other support with regard to telehealth.

DESCRIPTION OF RESPONDENTS:

Participants of BHW Loan Repayment and Scholarship Programs using the program portal website at https://programportal.hrsa.gov/.

FEDERAL COST:

The estimated annual cost to the Federal government is $2,000.00 which includes 1) $1,200 in configuration and testing, and 2) $800 (12 hours at the GS-14 level) in project management and oversight.

TITLE OF INFORMATION COLLECTION: Provide the name of the collection that is the subject of the request. (e.g. Comment card for soliciting feedback on xxxx)

PURPOSE: Provide a brief description of the purpose of this collection and how it will be used. If this is part of a larger study or effort, please include this in your explanation.

DESCRIPTION OF RESPONDENTS: Provide a brief description of the targeted group or groups for this collection of information. These groups must have experience with the program.

TYPE OF COLLECTION: Check one box. If you are requesting approval of other instruments under the generic, you must complete a form for each instrument.
