Individuals and Households Program (IHP), has explored alternative inspections methods, such
as geospatial inspections and desktop inspections, in order to deliver assistance efficiently. As a
result, many applicants will not receive a traditional on-site inspection, where applicants would
normally sign the FEMA Form 009-0-3, Declaration and Release prior to receiving IHP
Assistance.
The information from FEMA Form 009-0-3 has been added to the Disaster Assistance
Registration Privacy Act Screen. This will require the applicant to certify their acknowledgment
of the terms in Declaration and Release 009-0-3 in lieu of a signature, as they previously
accepted the Privacy Act Notice and Declaration of Eligibility under penalty of perjury.
