Appendix D. 2020 FDA CTP Emails Survey Informed Consent

Welcome to the 2020 FDA Center for Tobacco Products (CTP) Emails Survey!

You must be 18 years of age or older to be eligible to complete this survey.

The purpose of this 2020 FDA CTP Emails Survey is to see how satisfied you are with CTP’s e-newsletters (i.e., CTP Connect, CTP News, Spotlight on Science, and Modified Risk Tobacco Product Application Updates), and for us to learn a little bit about you.

This 5-minute, anonymous, survey is completely voluntary, and you may quit, without penalty, at any time. We anticipate up to 1,200 individuals will respond to this survey. Your data will be reported at the aggregate level. Respondents’ names, email addresses, IP addresses, and other personal identifying information will not be collected. If you feel uncomfortable answering any questions, you may choose the “prefer not to answer” response option. While there are no direct benefits to your participation in this survey, your feedback will help us improve our email communications.
