Notice on September 18, 2015 (80 FR
56550). VA also published the
Independent Assessment Report on its

Since then, VA has simplified the
website content to give Veterans easier
access to practical information about
eligibility and participation in the
Veterans’ Choice Program. As part of
this redesign, VA moved the
Independent Assessment Report to
https://www.va.gov/HEALTHPOLICY
PLANNING/section1.asp. This Federal
Register Notice informs the public of
this change so that Veterans and other
stakeholders will continue to have
access to these important resources.

Type of Review: Re-instatement of a
currently approved collection.
Abstract: VA Forms 21–8951 and 21–
8951–2 are used by reservists/
guardsmen to file a waiver a VA
disability benefits in order to receive
active or inactive duty training pay.
An agency may not conduct or
sponsor, and a person is not required to
respond to a collection of information
unless it displays a currently valid OMB
control number. The Federal Register
Notice with a 60-day comment period
soliciting comments on this collection
of information was published at 82 FR
213 on November 06, 2017, page 51482.

hours.

Previously Approved Collection.
Abstract: These forms are used by
veterans to authorize the Department of
Veterans Affairs (VA) to make
deductions from benefit payments to
pay premiums, loans and/or liens on
his/her insurance contract. The
information requested is authorized by
law, 38 CFR 8.8. This form was allowed
to expire due to high level of work
volume and staffing changes.
