Screener Consent
Hi, my name is ____________. I am a Research Coordinator at Northwestern University in the
Department of General Internal Medicine working with Dr. Michael Wolf. We are doing a
research study about over the counter medications. Do you have a few minutes for me to briefly
tell you about the study to see if you are interested in participating?
The name of this study is “The Influence of Brand and Principle Display for Safe Consumer use
of Over the Counter Drugs” The study number is STU00202120. This study is being done in
order to understand how consumers identify and choose over-the-counter medications. If you
choose to participate we will ask you questions about your over the counter medication use and
knowledge. There is one in-person interview that will take about 45 minutes. We will pay you
$30 for your time and effort.
