As you are aware, based on the evaluation report sent on mm/dd/yyyy, our analysis of your organization’s XX data showed that your organization had not met the minimum requirements to retain CDC recognition found in the Diabetes Prevention Recognition Program Standards and Operating Procedures (DPRP Standards).

Regrettably, because your organization did not meet these minimum requirements, your organization has lost CDC recognition and will be removed from CDC’s Registry of All Recognized Organizations.

There is a mandatory 6-month waiting period before your organization may reapply for CDC recognition. If, after that time, your organization wishes to reapply, we would welcome the application and renewed participation in the program. We encourage organizations wishing to reapply to use the waiting period as an opportunity to review the DPRP Standards and recognition requirements and reassess organizational capacity for program delivery.
