I am contacting you because you have been identified as a clinic champion in Clinic [insert name] and your clinic participated in a multi-centered study funded by the Centers for Diseases Control and Prevention (CDC) (PIs Mathew Samore, MD and Karl Madaras-Kelly, PharmD). We’d like to invite you to participate in a survey as part of this study. Your feedback is of critical importance to us.
This survey will solicit your opinions on the ARI Campaign elements and suggestions for improvement. Your responses will be used to refine delivery of the ARI Campaign. Any identifiable information submitted by you will only be available to the research team. All results shared with local facility investigators or in final reports will be de-identified and reported in aggregate.
