Subject: Online Video Interview for FDA Prescription Drug Labeling Study



Dear <NAME>:

Thank you for agreeing to participate in the interview for the research study assessing healthcare providers’ understanding and use of prescription drug labeling. Detailed in this email are some general instructions, followed by specific information needed to participate in the interview.

Your interview is scheduled to take place at the following date/time <DATE>.

The interview is scheduled for 60 minutes, therefore please be sure to allocate enough time in your schedule to accommodate the entire session.

As a token of our appreciation, a monetary incentive will be issued for participating in the interview. You can typically expect to receive a check in the mail within 4 to 6 weeks after you have completed the discussion.

The interviews will be conducted online using Zoom, a video and audio platform. You can access Zoom from any web browser using the link listed below. You will also need a phone for the audio portion of the interview. After you login into Zoom, you will be given the option of inputting your phone number for Zoom to call you, or you can call the phone number listed. You will then be connected with the interviewer.
