CMS Tribal LTSS Program Survey

Greetings:

On behalf on the Centers for Medicare and Medicaid (CMS), Kauffman and Associates, Inc. (KAI) is
writing to ask for your help with the CMS Tribal Long Term Services and Supports (LTSS) Program Survey.
CMS is an agency of the U.S. Department of Health and Human Services with a mission to effectively
steward public funds and strengthen and modernize, “the nation’s health care system to provide access
to high quality care and improved health at lower cost”. The purpose of the survey is to develop a list of
tribally operated LTSS programs across Indian Country and promote the sharing of LTSS best practices
across Indian Country.
The questionnaire is short, only 28 questions, and should take about 15 minutes to complete. To begin
