Minor Revisions to the Collection Instrument
SSA is making the following revisions:
Change #1: In the “CHECK BLOCK 1 AND THE STATEMENTS THAT APPLY OR CHECK BLOCK 2” section, we are removing the words “face-to-face” from option #1.
Justification #1: We are removing these words as we conduct hearings in-person as well as through video teleconferencing. We are removing these words as they imply an in-person hearing.
Change #2: We are revising the Privacy Act Statement on this form.
Justification #2: SSA’s Office of the General Counsel is conducting a systematic review of SSA’s Privacy Act Statements on agency forms. As a result, SSA is updating the Privacy Act Statement on the form.
