T ITLE OF INFORMATION COLLECTION: 2019 Low Income Home Energy Assistance Program (LIHEAP) Grantee Conference Feedback

PURPOSE: To get quick, written feedback from 2019 Low Income Home Energy Assistance Program (LIHEAP) grantees that attend the LIHEAP grantee training conference (3 days) hosted by the Administration for Children and Families (ACF). The feedback relates to the adequacy of the meeting space and the quality of the presentations and facilitation to help us assess how to improve the learning experience for our grantees at the following year’s training conference. Feedback will be requested on the overall conference (Attachment A), facilitators (Attachment B), and sessions (Attachment C).

DESCRIPTION OF RESPONDENTS: Primary LIHEAP grantees, e.g., representatives from state, tribal and territory government agencies that administer the federal LIHEAP funds.

We have a registration list of those LIHEAP grantees that have registered, for free, to participate in the conference. All conference attendees will be provided the feedback forms and their completion of the forms is voluntary.

TITLE OF INFORMATION COLLECTION: Provide the name of the collection that is the subject of the request. (e.g. Comment card for soliciting feedback on xxxx)

PURPOSE: Provide a brief description of the purpose of this collection and how it will be used. If this is part of a larger study or effort, please include this in your explanation.

DESCRIPTION OF RESPONDENTS: Provide a brief description of the targeted group or groups for this collection of information. These groups must have experience with the program.

TYPE OF COLLECTION: Check one box. If you are requesting approval of other instruments under the generic, you must complete a form for each instrument.

CERTIFICATION: Please read the certification carefully. If you incorrectly certify, the collection will be returned as improperly submitted or it will be disapproved.
