The survey is for recipients of technical assistance on worker safety and health from CDC/NIOSH as part of the CDC COVID-19
response. We want to know your thoughts about our virtual assistance so we can improve in the COVID-19 response and future
responses.
The survey is voluntary and will take about 12 minutes. The information you provide will be used for program improvement purposes
only, will not be shared in any way that would identify you or your organization. NIOSH will keep information you provide private and
secure to the extent permitted by law.

Public reporting burden of this collection of information is estimated to average 12 minutes per response, including the time for reviewing instructions, searching
existing data sources, gathering and maintaining the data needed, and completing and reviewing the collection of information. An agency may not conduct or sponsor,
and a person is not required to respond to a collection of information unless it displays a currently valid OMB control number. Send comments regarding this burden
