The survey is part of the National Evaluation of the Performance Partnership Pilots for Disconnected Youth (P3) sponsored by the U.S. Department of Labor. If you have already completed the survey, please accept our sincere appreciation! If you have not yet completed your survey, please do so at your earliest convenience but no later than <date>.
