Explanation of Non-Substantive Changes to OMB Control Number: 3060-0589:

• FCC Form 159

• Instructions FCC Form 159

Purpose of this Submission: The Commission is seeking a non-substantive change request for this information collection. This submission seeks to update FCC Form 159 and the accompanying instructions to FCC Form 159. Please see the attached updated form and instructions.

Summary of Proposed Change:

• Clarifications: Clarifies that FCC Form 159 and FCC Form 159-C (Continuation Sheet) is only to be used to accompany non-electronic payments to the Federal Communications Commission (as opposed to “any payment”), i.e., those by check, cash, or money order deposited through one of the Commission’s lockboxes. Electronic payments (such as payments by credit card or bank transfer) are to utilize the on-line form generated by the electronic payment system reached through the Commission’s website, known as FCC Form 159-E. As the Commission is in the process of sequentially closing its bank boxes and hereafter requiring only electronic payments, this change will clarify that this form is only needed during the submission of a manual payment during this transition.

• Corrections: Updated contact information, such as web links, that have changed since the form was last modified.

• Formatting: Fixed minor formatting errors contained in the prior iteration of the instructions to the form, such as inadvertent font changes and inconsistent line breaks.

• There are no changes to the burdens and there are no costs are associated with this collection.
